What is isotretinoin? What is Accutane®?

Accutane® is the most popular brand name for a drug called isotretinoin. It is one of the vitamin A derivatives from the family called retinoids. We use isotretinoin to treat severe forms of acne. Accutane® comes as jelly capsules, and you can buy it in doses of 10, 20, and 40 mg of isotretinoin in one capsule. Other brands of isotretinoin also come in a 5 mg dose.

How long does Accutane® take to work? Treating acne with the right Accutane® dosage takes time as the therapy usually lasts for months or even more than a year. However, it is supposed to treat your skin for the rest of your life. Sometimes, in cases of relapse, the course can be repeated.

How to use the isotretinoin dose calculator

The isotretinoin dose calculator can be helpful with a few problems regarding Accutane dosage. It has a built-in Accutane® dosage chart. To use it, follow the below instructions:

  1. Enter your body weight. You can switch between kg (metric) and lbs (imperial/American).
  2. Choose a calculation mode:
    • Enter daily dose → get duration: provide your daily dose in mg/kg/day, and the calculator will compute how long your therapy should last to reach the target cumulative dose.
    • Enter duration → get daily dose: provide the planned duration in months, and the calculator will compute the required daily dose.
  3. Set the cumulative target dose (mg/kg). The standard range is 120–150 mg/kg, with a mean of 135 mg/kg. Your dermatologist may adjust this.
  4. You will then see your daily dose in mg, the treatment duration in weeks and months, and the total cumulative dose for the min/mean/max range.

How to set a daily dose

You usually start with a low introductory dose, and if you feel okay, you can increase it to around 1 mg/kg per day — the recommended maximum dose for most people. However, singular severe cases of adult acne can benefit from increasing it to even 2 mg/kg per day. However, the cumulative isotretinoin dose does not change. Your leading doctor should always have the last word in terms of your daily and cumulative dose.

How does Accutane® work?

There are three main mechanisms of acne:

  • Sebum overproduction — If you happen to have oily skin or oily hair, you know what we are talking about;
  • Presence of specific bacteria — Such as Propionibacterium acnes; and
  • Intense keratinization — This means that the skin cells produce more of the protein keratin than usual. It thickens our skin and traps the oil and bacteria in the skin, causing inflammation and pimples.

Oral isotretinoin is the only drug that works on every level:

  • It promotes cell turnover so that keratinization slows down.
  • It shrinks the sebaceous (oil) glands so they don't produce as much sebum.
  • It stops acne bacteria from multiplying, resulting in less inflammation and reduced redness.

How to reduce the side effects of isotretinoin

Every drug has possible side effects, and Accutane® is no exception. The drug is quite potent and can cause some complications. During treatment, you should check your blood often, be mindful of any symptoms, and if you're a woman, take care of effective contraception.

More common side effects

  • Dryness of the lips and skin;
  • Dry eyes;
  • Skin sensitive to sunlight — every retinoid makes your skin very sensitive to sunlight; using a high SPF product is a must!
  • Dry nose and nosebleeds;
  • Dry throat and mouth; and
  • Headache, back pain, joint pains.

Less common, but more severe side effects

  • Birth defects if taken by a pregnant woman — always stay on effective contraception while taking isotretinoin;
  • Fatigue;
  • Changes in blood lipids — increase of LDL and total cholesterol, increase of triglycerides that can potentially cause pancreatitis. Take blood tests regularly;
  • Liver damage and an increase of liver enzymes AST and ALT;
  • Blood sugar increase;
  • Changes in blood test — e.g., anemia; and
  • Mood drop and depression.

Tips to reduce side effects

  • Apply rich lip balm frequently throughout the day.
  • Use gentle, fragrance-free moisturizers on your face and body.
  • Apply SPF 30+ sunscreen daily, even on cloudy days.
  • Use lubricating eye drops if you experience dry eyes.
  • Stay well-hydrated and avoid alcohol.
  • Take isotretinoin with a fatty meal to improve absorption.
  • Get regular blood tests as directed by your dermatologist.

How to manage your therapy with Accutane®

Successful isotretinoin therapy requires close monitoring by a qualified dermatologist. Key management steps include:

  • Initial labs: Complete blood count, liver function tests, and fasting lipid panel before starting.
  • Monthly monitoring: Repeat blood tests monthly throughout treatment.
  • Pregnancy prevention: Two forms of contraception are required for women of childbearing age, both during therapy and for 1 month after stopping.
  • Avoiding vitamin A supplements: These add to the retinoid effect and can cause toxicity.
  • No blood donation: Do not donate blood during treatment or for 1 month after stopping.
  • Follow-up after completion: Acne may continue to improve for up to 2 months after the course ends.

FAQs

What is the typical cumulative isotretinoin dose?

The standard cumulative dose target is 120–150 mg/kg of body weight, with a mean of 135 mg/kg. Lower cumulative doses (below 120 mg/kg) are associated with a higher risk of relapse.

How long does an Accutane® course last?

A typical course lasts 4 to 9 months, depending on the daily dose and the target cumulative dose. With a dose of 1 mg/kg/day and a target of 135 mg/kg, the course would last approximately 4.5 months (≈ 20 weeks).

Can I take isotretinoin at a lower dose?

Yes. Low-dose regimens (0.25–0.5 mg/kg/day) are sometimes used for patients with mild-to-moderate acne or those who are sensitive to side effects. However, to achieve the same cumulative dose, the treatment duration is longer.

Is this calculator a substitute for medical advice?

No. This calculator is for informational purposes only. Your dermatologist will determine the appropriate dosage based on your individual health status, skin condition, blood tests, and other factors. Always follow your doctor's prescription.
